New Perk: Estate Planning

CMDR Andre' Haynes is proud to announce that a New Member Perk is now available.

Active members of the Armed Forces Chamber of Commerce are now eligible to receive FREE estate planning documents, document preparation, notarization and registration courtesy of Patsy Brown, JD. This new member perk is valued at $1,500.

Documents include a basic Will, Medical Power of Attorney, Financial Power of Attorney, Revocable and Irrevocable Trusts. Documents are prepared by CMDR Andre' Haynes, a Nevada licensed document preparer (License Number: NVDP202111175182). Documents are reviewed by Patsy Brown, JD, a contract lawyer. Documents are notarized by Andre' Haynes (Certificate No. 10-1975-1), a Nevada notary and Nevada Signing Agent. Documents are registered with Nevada Lockbox via the Nevada Secretary of State.

Medical Power of Attorney

In the field of medicine, a healthcare proxy is a document with which a patient appoints an agent to legally make healthcare decisions on behalf of the patient, when the patient is incapable of making and executing the healthcare decisions stipulated in the proxy.

Financial Power of Attorney

A financial power of attorney form allows a person to choose someone else to handle their financial decisions and affairs. The form allows the person granting authority (“principal”) to choose someone else (“agent”) to have the same powers as if they were acting themselves.


Revocable Trust

A trust that can be modified or dissolved without the permission of the beneficiary. During the life of the trust, income from the corpus is distributed to the grantor. Transfer of assets to beneficiaries only occurs at the time of the grantor's death.

Nevada Lockbox

The Nevada Lockbox is an electronic registry securely maintained on the Nevada Secretary of State website, which contains an electronic reproduction of each document filed by the registrant. The Nevada Lockbox has two filing components: the Advance Directive Registry and the Guardianship Nomination Registry. The Advance Directive Registry is a simple and secure approach to ensure that your medical wishes are followed. A copy of your advance directive will be kept confidentially and readily available to you and your health care provider, when needed, 24-7. The Guardianship Nomination Registry is a secure filing of your "Request to Nominate Guardian Form" which is available in the event that an authorized entity needs to determine whether a person has nominated a guardian.
